2. Services Provided

Wellmind Health provides outpatient mental health services, including psychiatric evaluation, medication management, and related administrative and digital services. Services may include in-person and/or telehealth encounters where permitted by law.

Use of this website does not establish a provider-patient relationship until formal intake, informed consent, and confirmation by Wellmind Health.

3. Eligibility

You must be:

  • At least 18 years old, or

  • A minor receiving services with legally valid parental/guardian consent

  • Physically located in Utah at the time of service unless otherwise authorized

We reserve the right to refuse service where legally required.

4. Telehealth Consent

By using telehealth services, you acknowledge and agree that:

  • Telehealth involves electronic communication of health information

  • Technical failures may occur

  • Telehealth may not be appropriate for all conditions

  • Emergency services should be accessed via 911 in urgent situations

Telehealth is provided in compliance with applicable federal and Utah state laws.

5. No Emergency Services

Wellmind Health does not provide emergency or crisis services.

If you are experiencing:

  • Thoughts of self-harm or harm to others

  • A medical or psychiatric emergency

Call 911 or contact the 988 Suicide & Crisis Lifeline immediately.

6. Communications & SMS Consent

By providing your phone number or email address, you consent to receive:

  • Appointment reminders

  • Scheduling communications

  • Administrative and care-related messages

Message and data rates may apply. You may opt out of SMS communications at any time by replying STOP.

We do not guarantee uninterrupted or error-free electronic communications.

7. Protected Health Information (HIPAA)

Use of our services may involve the collection and use of Protected Health Information (PHI).

PHI is handled in accordance with HIPAA and our Privacy Policy and Notice of Privacy Practices, which are incorporated into these Terms by reference.

8. Payment & Insurance

You agree to:

  • Provide accurate billing and insurance information

  • Pay all applicable fees, copays, deductibles, or self-pay charges

  • Understand that insurance coverage is not guaranteed

Failure to pay may result in suspension of services or referral to collections where permitted by law.

9. Cancellations & No-Shows

Appointment cancellations must be made within the timeframe specified in your intake documents. Late cancellations or no-shows may result in fees not covered by insurance.

10. Website Use & Intellectual Property

All content on this website, including text, logos, graphics, and design, is the property of Wellmind Health PLLC and may not be copied, reproduced, or distributed without written permission.

11. Third-Party Platforms

We may use third-party platforms (e.g., scheduling systems, EHRs, CRM tools) to deliver services. We are not responsible for service interruptions or issues caused by third-party systems.
